Personally, I think it is poor for the grandson to call you by first name. However, if that is what the family has "introduced" you as instead of grandma, then I would leave it.
My 1/2 brother has two children that call me "Auntie", but call my mother by her first name or "Grandma (first name)".
If it really bothers you, I would bring it up to your husband. You are a part of the family and have the right to be called Grandma even though he is your step-son's son. Your husband should be able to understand your feelings and try to figure something out with you.

I prefer the tradition of "titles".

Personally, I prefer "titles", i.e. for you to be called "Grandma" by your husband's son's child.

I like titles because maybe I like that tradition. To me, it feels like a sign of respect of elders.

But I'm also aware that there are different personal preferences.

Some don't like to be called "Grandma" because it makes them feel old, some like it, because they feel respected. Many enjoy the compliments, "Wow, she's your grandma? She is so young!"

So if you would like to be called Grandma, it is better to start making your preference known as early as possible.
